Family Medicine Physician Outpatient | Nonprofit FQHC (Northern Indiana)

What you ll do

• Provide full-spectrum outpatient primary care in a team-based, PCMH environment
• Monday Friday, 8a 6p with one late evening to 8p; no weekends
• After-hours nurse triage handles initial calls; no hospital/ED call
• Supported by RNs/MAs, on-site behavioral health, pharmacy, and athena EMR

Compensation & benefits

• Pay: up to $245,000/year
• Incentives: Up to $18,000 sign-on or relocation
• Loan repayment: Federal program eligible
• Coverage/Time: FTCA malpractice; 5 CME days + $2,500; ~4 weeks PTO + 11 holidays
• Retirement/Health: 403(b) with 3% match; medical/dental/vision
• Other: No non-compete

Qualifications

• MD/DO; BE/BC in Family Medicine
• Indiana license & DEA (or ability to obtain)
• Visa sponsorship not available

Location

• Northern Indiana, commutable to the South Bend metro; multi-site community health center
